His clearest message is about gene therapy — and a distinction most people miss. There is gene editing (CRISPR), which alters the chromosome, and there is gene transfer therapy, which simply adds a working gene to a cell so it makes the protein you need. His memorable, deliberately silly example: if you’re going bald, you could add “a gene that makes your hair grow.” Gene transfer, he argues, is the safe route — it avoids the things people fear, because what you add isn’t passed down the germline.
One gene he keeps coming back to is Klotho — a candidate he calls a possible “silver bullet.” He’s careful, though: he doesn’t think there will be a single magic therapy, but a number of silver bullets, some better than others, unfolding over time.
And he points to what comes next: epigenetic reprogramming — “the next step beyond gene therapy,” which he expects to come online within a few years. Not slowing ageing, but reversing it: literally undoing the damage that ageing has done. That is the whole promise of this level of the map, spoken plainly by someone who has spent a career getting there.
We filmed Patrick in London, in a room with a piano and a lifetime of art on the walls. He is unhurried and precise — the kind of doctor who has been “always on the cutting edge” long enough to explain the frontier without hype. He walked us through what gene therapy actually is, why the transfer approach is safe, and why reprogramming is the thing to watch.
